宋代：释遵式
稽首西方安乐刹，弥陀世主大慈尊。我依种种修多罗，成就往生决定信。
住大乘者清净心，十念念彼无量寿。临终梦佛定往生，大宝积经如是说。
五逆地狱众火现，值善知识发猛心。十念称佛即往生，十六观经如是说。
若有欢喜信乐心，下至十念即往生。若不尔者不成佛，四十八愿如是说。
诸有闻名生至心，一念回向即往生。唯除五逆谤正法，无量寿经如是说。
临终不能观及念，但作生意知有佛。此人气绝即往生，大法鼓经如是说。
一日一夜悬缯盖，专念往生心不断。卧中梦佛即往生，无量寿经如是说。
昼夜一日称佛名，殷勤精进不断绝。展转相劝同往生，大悲经中如是说。
一日二日若七日，执持名号心不乱。佛现其前即往生，阿弥陀经如是说。
若人闻彼阿弥陀，一日二日若过等。系念现前即往生，般舟经中如是说。
十日十夜六时中，五体礼佛念不断。现见彼佛即往生，鼓音王经如是说。
十日十夜持斋戒，悬缯幡盖然香灯。系念不断得往生，无量寿经如是说。
若人专念一方佛，或行或坐七七日。现身见佛即往生，大集经中如是说。
若人自誓常经行，九十日中不坐卧。三昧中见阿弥陀，佛立经中如是说。
若人端坐正西向，九十日中常念佛。能成三昧生佛前，文殊般若如是说。
我于众经颂少分，如是说者无穷尽。愿同闻者生正信，佛语真实无欺诳。

释遵式（九六四～一○三二），俗姓叶，字知白，天台宁海（今属浙江）人。少投东山义全出家，太宗雍熙元年（九八四）从宝云义通受业。淳化初，居宝云讲席。真宗咸平中，归天台。大中祥符中，历居景德寺、杭州昭庆寺讲席。后居复兴故天竺寺，赐号慈云。著《净土忏法》、《金光明》、《观音》诸本忏仪行世，又号慈云忏主。仁宗明道元年卒，年六十九。
