稽首宝陀山，观音大菩萨。三十二妙应，垂福于人天。
慧日廓圆明，隐微无不照。我等由宿植，于菩萨有缘。
曩善一毫端，菩萨悉能鉴。今兹一何幸，联樯来宝山。
淑景德惠风，沧海如鉴净。朝发东秦岸，须臾达莲宫。
僧饭洗钵时，相与瞻远峤。□□跨巨浪，侧足骨毛□。
自念尘坌身，得伴清净侣。同声讽密语，复念我师名。
注目渴慈光，忽睹紫金相。崆峒石窟上，宴坐自在容。
云雾不蔽亏，跪礼得良久。偕来无老稚，咸识应现尊。
赞叹且忻惧，得所未曾有。我念现前众，中多大福人。
久新迎我师，所祷如响应。为我忏众恶，遂亦见如来。
同办起心香，复陈一日供。倘以昔善积，获此大吉祯。
愿敦未来因，永托如来记。浮生如梦幻，孰处放教生。
戒杀多素羞，广阅如来藏。分阴实可惜，栖迟必山林。
料理般若舟，长想菩提岸。群生迷苦海，拯援使知津。
共发向上心，无□同佛会。

（1083—1145）宋福州永福人，字德邵。徽宗崇宁五年进士。历官监察御史、殿中侍御史，钦宗时三上章劾秦桧专主和议，植党专权，遂夺桧职。累迁起居舍人、中书舍人兼给事中。司谏詹大方希桧意，劾龟年附丽匪人，落职归籍。
